Directions for Requesting a Charitable Gift from Your IRA or Roth IRA

1. Display and print a copy of the Request for Qualified Charitable Distribution.
2. Fill out the form completely.
3. Make two extra copies of the form.
4. Send the original copy to your IRA or Roth IRA Plan Trustee/Administrator, keep a copy for your records, and send the third copy :

5. Once The Corneal Dystrophy Foundation  has received your gift from your plan trustee/administrator, we will provide you with a written acknowledgment that your gift has been received.
